This form serves as a contract for Simple Simon Bakery to provide desserts for your wedding. This form includes all contact information and product / design files for a wedding cake or wedding desserts as listed for the specified date. A minimum of a $25 non-refundable deposit secured this date, with the balance due on this order paid in full one week prior to the wedding date. All other information will be listed on another wedding form to include details and a final balance.
After submitting this form, you will be directed to pay your deposit securely online. A minimum $25 non-refundable deposit is required to secure your date.
